* At least 17 years old of age

* Understand & able to speak English

* At least High School Graduate or equivalent

* Passport photograph size 2x3 ; 3x4 ; 4x6 ; four copies each with red backdrop

* Copies of self ID card & parent’s ID Card

* Copy of latest Education Certificate

* Declared Healthy by a Certificate of Medical second class.

* Invest a Registration Fee of USD 1,000 in the following bank account :

           a. Bank Mandiri Branch Plaza Dua Mutiara

                 - USD account No. 1240006549340

           b. Bank Mandiri Branch Puri Sentra Niaga

                 - IDR account No. 1660000812586

* Completion of Registration Form with Bank Receipt of registration Fee as attachment.
